Некоторые вопросы первичной и вторичной стабильности дентальных имплантатов

Введение.
На данный момент не сформировано единое мнение, действительно ли высокая первичная стабильность чаще приводит к остеоинтеграции, и
наоборот, какие цифры первичной стабильности приводят к дезинтеграции имплантата.
Цель
– оценить клинические результаты реабилитации стоматологических пациентов с помощью дентальных имплантатов.
Материалы и методы.
Были проанализированы отдаленные результаты установленных по стандартному двухэтапному протоколу 149 имплантатов у
41-го пациента в течение шести лет. Исследование включало клинические методы обследования, пальпацию, перкуссию, динамометрию, анализ резо-
нансной частоты колебаний.
Результаты.
Был проведен поиск закономерностей между первичной и вторичной стабильностью имплантатов. Величина усилия при установке влияла на
первичную стабильность, но в течение шести лет было отмечено нарушение вторичной стабильности 9-ти имплантатов (6,1 %), которые были удалены.
Величина усилия при установке имплантатов которые были удалены, была в среднем на 4,1 Нсм выше (39,5 Нсм по сравнению с 35,6 Нсм). Высокий кру-
тящий момент при установке имплантата может быть фактором нарушения остеоинтеграции в отдаленном периоде.
Выводы.
Высокий крутящий момент при установлении имплантата не только не приводит к повышению успеха остеоинтеграции, но и может быть факто-
ром, который в дальнейшем приводит к нестабильности имплантата.

Some questions of primary and secondary dental implants stability

Introduction.
At the moment, it is not yet formed a consensus, whether the increasing of high primary stability leads to osseointegration, and vice versa, what values
of primary stability of the implant lead to disintegration.
Purpose.
To evaluate the clinical results of the rehabilitation of dental patients with the help of dental implants.
Materials and Methods.
Long-term results were analyzed according to the standard established by the two-stage protocol 149 implants in 41 patients for 6 years.
The study included: clinical methods of examination, palpation, percussion, dynamometer, analysis of the resonance frequency.
Results.
Searches were conducted of laws between the primary and secondary implant stability. The amount of force when installing a primary effect on stability, but it
was noted violation secondary stability of implants 9 for six years (6.1 %), which were removed. The amount of force when installing the implants have been deleted, it
has an average of 4.1 Ncm above (39.5 Ncm compared to 35.6 Ncm). High torque at the implant may be a factor in violations of osseointegration in the long term.
Conclusions.
High torque at the implant establish not only increases the success osseointegration, but also may be a factor which further leads to instability of the implant.
