Бугорно-крыловидные имплантаты – терапевтическая опция для пациентов с дефектами
дистальных отделов верхней челюсти

Резюме.
Дистальные отделы верхней челюсти традиционно считаются неблагоприятными для проведения дентальной имплантации, что связано, в пер-
вую очередь, с неадекватным количеством и качеством костной ткани, топографической близостью верхнечелюстного синуса и т.п. На этапе передим-
плантационной подготовки в этой области, как правило, используются дополнительные хирургические процедуры с целью увеличения костного объема, что
приводит к увеличению стоимости и времени протезной реабилитации пациента.
Исторически для достижения необходимого объема костной ткани в дистальных отделах верхней челюсти использовался целый ряд хирургических вме-
шательств, включая частичную или полную остеотомию верхней челюсти, костную трансплантацию и т.п., наибольшего распространения среди которых
получили операции «открытого» и «закрытого» синус-лифта.
С тех пор, когда J.F. Tulasne в 1985 году описал оригинальную методику установки бугорно-крыловидных имплантатов, использование мощных кортикаль-
ных областей крыловидного отростка стало предметом практического интереса для многих специалистов имплантологического профиля, и, в особенности,
работающих в сфере кортикальной имплантации.
В работе отображены результаты собственного опыта использования бугорно-крыловидных имплантатов у 12 пациентов, коротка описана техника их уста-
новки и приведены клинические примеры.

Summary.
Distal maxillary aspects traditionally are considered unfavorable for dental implantation due to non-adequate quantitative and qualitative bone character-
istics, topographic vicinity of maxillary sinus etc. On the pre-implant stage of patient’s rehabilitation additional surgical procedures aimed to increase a bone volume,
usually cost- and time-consuming, are carried out.
Historically, a plethora of surgical techniques have been proposed to restore the atrophic posterior maxilla, including partial or complete osteotomies, bone grafting
etc, but sinus floor elevation became one of the most popular procedure.
Since J.F. Tulasne in 1985 described the original technique of placement of pterygoid implants, the engagement of strong cortical areas of pterygoid plates became
a matter of interest for many dental practitioners, especially those working in the field of cortical implantology.
In this scientific work the results of our own clinical experience of employment of tubero-pterygoid implants in 12 patients are displayed, the technique of their instal-
lation is briefly described and clinical examples are given.
